Job Opening Summary

 

The Associate Vice President of Patient Care Transitions will lead and oversee the strategic planning, development, and execution of initiatives aimed at optimizing patient care transitions across the healthcare continuum. The incumbent will play a pivotal role in ensuring seamless transitions of care for patients within UF Health Shands and its affiliated facilities, ultimately enhancing patient outcomes, satisfaction, and operational efficiency.

Job Opening Qualifications

 

Minimum Education and Experience Requirements:
• Master's degree in healthcare administration, business administration, nursing, or a related field.
• Minimum of 10 years of healthcare management leadership experience, with a focus on care transitions, post-acute care management, and care coordination.
• Strong leadership skills with the ability to build and lead high-performing teams.
• Proven track record of developing and implementing successful care transitions programs.
• Demonstrated ability to work collaboratively with clinical and operational leaders across a complex healthcare organization.
• Excellent communication and interpersonal skills with the ability to communicate effectively with stakeholders at all levels.
• Strong analytical and problem-solving skills with the ability to develop and implement metrics to measure the effectiveness of care transitions.
• Knowledge of healthcare regulations and policies related to care transitions and post-acute care management.
• Experience working in an academic medical center or other complex healthcare organization preferred.
• Experience with case management, social work, SNFs, rehab programs, etc., is preferred.
