Function effectively as a Florida Chapter 397 qualified professional or Florida license-eligible psychotherapist in substance use disorders and as a member of the FRC interdisciplinary, physician-led team specializing in the evaluation and treatment of individuals with substance use disorders and their families. Manage all aspects of assigned caseload including initial and ongoing assessment of clinical needs, develop and implement individualized treatment plans, develop and implement patient and family engagement strategies, anticipate and prevent early withdrawal from treatment, foster good long-term recovery outcomes by facilitating continuity-of-care including partial hospitalization, intensive outpatient, level 1 outpatient, Phase 4, sober living, engagement in community-based mutual-help fellowships and FRC alumni society affiliation. Initiate, facilitate and manage communication with referral source professionals and other collateral professionals involved in assigned cases, as delegated, including referring physicians, therapists, monitoring programs, attorneys, EAPs, judges and probation officers, etc. Perform effectively as an individual, group and family therapist under clinical supervision. Develop and provide therapeutic and educational activities utilizing individual, family and group modalities. Provide on-call coverage on a rotating basis with other therapists. Assures patient¿s rights are protected including HIPPA and CFR 42 confidentiality. Writes and sends high quality and timely reports to collateral professionals as authorized by the patient. Develops and implements comprehensive discharge plans/continuity of care plans in concert with FRC treatment team and collateral professional. Documenting patient care in accordance with Joint Commission, ASAM and hospital standards. Work with a high degree of professionalism and some independence. Participate productively in individual and group clinical supervision, demonstrating consistent professional growth and progress toward the next level of credentialing based on eligibility.
